Job Description
Snowline Health's hospice team is seeking a dedicated, compassionate Medical Social Worker to join our mission-driven organization. If you’re passionate about supporting patients and families with exceptional, person-centered care, we’d love to hear from you.
Requirements
- Master of Social Work (MSW) degree from a graduate school accredited by the Council on Social Work Education
- Licensed Clinical Social Worker (LCSW) preferred
- Minimum of one (1) year of clinical geriatric experience required; hospice experience preferred
