Nurse Practitioner

Snowline Health is a non-profit organization serving the western slope of El Dorado County and the Greater Sacramento Region. Our Nurse Practitioner will be responsible for providing compassionate and comprehensive care to patients and their families, collaborating with Medical Directors and other care team disciplines.

Requirements

• Current valid NP California license
• Minimum of three years of clinical experience in geriatric, palliative, or primary care
• Proficient in medical terminology, EMRs, and HIPAA compliance
• Knowledge of palliative care and hospice preferred
• Experience in home-based care delivery preferred
• Strong clinical assessment and diagnostic skills
• Excellent interpersonal, communication, and organizational skills
• Ability to work independently and collaboratively as part of an interdisciplinary team
• Valid CA driver’s license with a reliable, insured vehicle for home visits
• Active and updated DEA/CURES credentials
• Knowledge of trauma-informed care principles preferred
