Nurse Practitioner (NP) ElderCare Program

Snowline Health is seeking a Nurse Practitioner (NP) to deliver comprehensive, person-centered primary and palliative care services to seniors through home-based clinical settings.

Requirements

• Current and valid California NP license
• Minimum 3 years’ clinical experience in geriatrics, palliative care, or primary care
• Proficiency in EMRs, HIPAA compliance, and medical terminology
• Experience in home-based care delivery preferred
• Strong assessment and diagnostic skills
• Excellent interpersonal, communication, and time management skills
• Ability to work independently and as part of an interdisciplinary team
• Valid CA driver’s license and insured vehicle for home visits
• Active DEA registration and CURES compliance
• Knowledge of trauma-informed care a plus
