Registered Nurse Case Manager (RN)

As a Registered Nurse Case Manager, you will deliver individualized, holistic support to patients in their homes and collaborate with a care team to develop a personalized plan of care.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
