(RN) Registered Nurse Case Manager

At St. Croix Hospice, we provide compassionate care to patients and their families through the end-of-life journey. As a Registered Nurse Case Manager, you'll coordinate individualized care plans, support families, and offer comfort with professionalism, empathy, and respect.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Must have reliable transportation, valid driver's license, and proof of auto insurance
