(RN) Registered Nurse Case Manager

Registered Nurse Case Manager position at St. Croix Hospice providing 1:1, whole-person care to patients and families in their homes, coordinating individualized care plans and supporting families through complex decisions.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
