Registered Nurse (RN) Case Manager

We are seeking a Registered Nurse Case Manager to provide 1:1 holistic support to patients and families in their homes, partnering with an interdisciplinary team to develop individualized care plans and ensure comfort with professionalism and empathy.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Reliable transportation, valid driver’s license, and proof of auto insurance
