Registered Nurse (RN) Case Manager

We are a hospice care organization that guides patients and their families through the end-of-life journey with compassion and dignity. We are looking for a Registered Nurse Case Manager to provide holistic support to patients and families in their homes.

Requirements

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Reliable transportation, valid driver’s license, and proof of auto insurance
