Job Description
We believe in compassionate care and are seeking a Registered Nurse Case Manager to deliver individualized support to patients in their homes, leading end of life conversations and ensuring symptom management with empathy and clinical excellence.
Requirements
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience required; hospice/home health experience preferred
- Must have reliable transportation, valid driver’s license, and proof of auto insurance
