Job Description
Strive Health is a healthcare organization transforming the kidney care system through early identification, engagement, and coordinated care. The Nurse Practitioner will deliver exceptional clinical care to patients and work as a team to improve outcomes and provide a patient experience.
Requirements
- Master’s degree in Nursing or a related field
- 2+ years experience as a Family Nurse Practitioner, Primary Care Nurse Practitioner, or internal medicine, cardiology, nephrology, or case management NP
- Current state-specific licensure
- National Board Certification
- Current DEA license or eligibility
- Current BLS or CPR certification
