Job Description
Strive Health is seeking a Nurse Practitioner (NP) to deliver advanced nursing care for patients with kidney disease and related complications. The NP will collaborate with interdisciplinary teams, provide patient care in various settings, and advocate for patient needs within a patient-centered care model. The role involves managing patient outcomes, data analysis, and ensuring high-quality patient experiences.
Requirements
- Master’s degree in Nursing or similar field.
- 2+ years experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or Internal Medicine, Cardiology, Nephrology, or Case Management experience.
- Current state-specific licensure.
- National Board Certification.
- DEA license required or eligible.
- Current BLS or CPR Certification.
- Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency <60 ms.
