Job Description
Strive Health is seeking a Nurse Practitioner to deliver advanced nursing care for chronic kidney disease (CKD) and other kidney-related conditions. The role involves managing patient care, collaborating with interdisciplinary teams, and advocating for the patient in a high-touch model. Requirements
- Master’s degree in Nursing or similar field
- 2+ years experience as a Family Nurse Practitioner (NP), Primary Care Nurse Practitioner (NP), or in internal medicine, cardiology, Nephrology or Case Management
- Current state specific licensure required
- National Board Certification
- Current Drug Enforcement Administration (DEA) license or eligible to obtain
- Current BLS or CPR Certification
- Internet Connectivity (Min Speeds)
