Social Services Designee

Conduct an admission interview with residents and families, identify areas of concern, develop treatment plans, and maintain contact with families. Assist with discharge planning and contribute to resident care conferences.

Requirements

• Full-time position, Monday - Friday
• Social Services Designee Certificate
• Previous experience in skilled nursing facilities is preferred
• Strong communication skills
• Ability to handle sensitive situations with compassion and confidentiality
