Telehealth Care Navigator

Synapticure is seeking a Telehealth Care Navigator to provide direct support to people affected by neurodegenerative diseases. The Care Navigator will oversee patient relationships, enrollment, documentation, and care experience, as well as develop care coordination team workflows and processes.

Requirements

• Bachelor’s degree in a related field
• 2+ years experience in direct case or care management in the healthcare setting
• Comfortable using technology to support members without in-person contact
• Excellent verbal and written communications, organizational skills, and interpersonal skills
• Understanding of how to use scheduling platforms and electronic health record systems
• Proficient in collecting member clinical and demographic data and documenting appropriately
