Requirements
- Conduct outreach in targeted neighborhoods
- Engage individuals encountered during outreach or referred by partner agencies
- Provide intensive emotional and practical support to participants
- Build collaborative relationships with local community-based organizations
- Link participants to providers for various services
- Advocate for participants in achieving their goals
- Participate in ongoing training, team meetings, and supervision
- Develop short-term, person-centered treatment plans
- Monitor and document participant progress
- Conduct ongoing assessments to support sustained community integration
