The Case Manager 2 provides staff support and direct client care, delivering comprehensive case management services, and working on a care team to support clients’ recovery from mental illness, substance use, and homelessness.
Requirements
- Associate’s Degree
- 2+ years related experience in the direct service and case management field
- Valid Connecticut Driver’s License
- Regular and predictable attendance is required
