
Job description
As a Case Manager for the Community Connect Program, you will provide holistic, person-centred case management support to people experiencing homelessness, at risk of homelessness, or living in low-cost or insecure accommodation with multiple unmet health needs.
Provide high-quality case management support including assessment, case planning, reviews, and case closure. Deliver holistic, sensitive support that responds to individual needs and enables clients to achieve case plan goals.
You will support clients to access and navigate services, community supports, and housing pathways. Build effective, empowering relationships that encourage self-determination and participation.
