Hospice Liaison / Registered Nurse - Home Health and Hospice

We are seeking a Hospice Liaison / Registered Nurse to lead the way in hospice and palliative care, coordinating and driving business growth while fostering collaborative relationships with key healthcare providers and community members.

Requirements

• Current RN licensure or compact licensure recognized by the State of Vermont
• Minimum 5 years of experience in the hospice field
• Hospice & Palliative Care Certification (or willingness to obtain within 1 year)
• Advanced training in Serious Illness Conversations (e.g., TalkVermont)
• Experience in sales, marketing, and liaison roles
