The University of Vermont Health Network - Home Health & Hospice, formerly the Visiting Nurse Association of Chittenden and Grand Isle Counties, is a nonprofit home health and hospice agency serving Vermonters for more than 110 years. We care for 5,000 individuals and families each year. Our programs span lifetimes -- from prenatal care to care for young families to rehabilitation, long-term care, adult day services to end of life. We provide care one person, one family at a time. With support from our neighbors, UVM Health Network - Home Health & Hospice provides medically-necessary home and community-based care to individuals and families regardless of their ability to pay. Each year, we provide more than $1 million in charitable care to our neighbors in need.

Personal Care Attendant - Long Term / Choices for Care Home Health
High school diploma or equivalent, completion of PCA coursework, and satisfactory standing with various registries
Registered Nurse - Night On Call - Community Hospice
4 years experience as Community Hospice Nurse or 5 years in home health nursing, Associate's degree in nursing or higher, must live within reasonable driving distance to service area
Registered Nurse - Adult Home Health
1+ year of home health or public health nursing experience
Hospice Liaison / Registered Nurse - Home Health and Hospice
5 years of hospice experience, RN licensure, and certification in hospice and palliative care
LNA - Home Health and Hospice
Vermont Nursing Assistant license, previous homecare or facility care experience, and ability to communicate effectively with clients
Licensed Practical Nurse (LPN) – High-Tech Home Health | Flexible Day or Night Shifts
Current LPN license, 2+ years of experience in home health, strong communication and clinical skills
