As a partner in building brighter futures for youth and their families, your role will be to help individuals and families navigate healthcare, behavioral health, social services, community systems, and support activities that align with The Village Network’s Mission, Vision & Values.
Requirements
- Conduct comprehensive assessments to identify clients’ needs and strengths.
- Develop, implement, and monitor individualized care plans with clients and their families.
- Facilitate care team meetings that include clients, providers, and support systems.
- Advocate and link clients to community resources, including, but not limited to, healthcare, behavioral health supports, housing, social services, and benefits that will assist clients in meeting their needs, helping to resolve barriers to successful service delivery.
- Maintain timely, accurate case documentation and compliance with TVN policies and regulations, along with ensuring adherence to agency, state, licensing, Ohio Rise, and Medicaid program rules and regulations.
- Provide care services in the following ways: Supporting the client’s efforts toward treatment goals, Identifying strengths and needs, Role model and teach appropriate skills and behaviors, Utilize a trauma-focused approach, Assist the youth with daily activities, Responsible for assisting with crisis de-escalation as needed.
