Job Description
The Clinical Case Manager provides clinical support to build brighter futures for youth and their families, implementing individualized treatment plans and facilitating case management. Excellent benefits, competitive salaries, and 192 hours of PTO offered in the first year.
Requirements
- Bachelor's Degree in Social Work, Counseling, or related field
- 3+ years of relevant experience
- Valid Ohio Driver's License
- Ability to use a computer, proficiency in Word and Electronic Health Record (EHR)
- Excellent verbal and written communication skills, strong teamwork and organization/time management skills
- Ability to handle physical demands, including bending, stooping, lifting, pushing, reaching, and walking for periods of time
