Job Description
The Village Network is seeking an OhioRISE CME Care Coordinator to cultivate flexible, family-focused, community-based responsive services based on the High-Fidelity Wrap Around Model of care coordination for Tier II Moderate clients.
Requirements
- Comply with all OhioRISE requirements of a CME Care Coordinators
- Develop and maintain the Wraparound Team
- Coordinate and supervise implementation of the Plan of Care
