Job Description

Our OhioRISE CME Care Coordinator role is responsible for cultivating flexible, family-focused services based on the High-Fidelity Wrap Around Model. As a member of our team, you will receive a $3,000 signing bonus, tuition and licensure reimbursement, and excellent benefits, including 232 hours of PTO in the first year.
Requirements
- Licensed Social Worker or equivalent
- High School Diploma/GED with 3 years of experience or Associate/Bachelor's degree with 2 years of experience or Master's degree or higher with 1 year of experience
- Experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related field
- Culturally competent with training and experience in managing complex cases
- Ability to use a computer and proficiency in Word and Electronic Health Record (EHR)
- Valid Ohio Driver's License and insurable driving record
