Job Description
Transitions Hospice is seeking a RN Case Manager to join their team, providing 24/7 care to patients and their families in hospice care. As an RN Case Manager, you will be the clinical anchor for your patients and their families, coordinating care and guiding the interdisciplinary team.
Requirements
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
- One (1) + year Hospice, home health or related organization experience preferred
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
