LVN Case Manager- Bakersfield 1.2

The LVN Case Manager will work with members with complex medical and social needs, providing comprehensive care management and coordination of services. The position requires strong interpersonal and organizational skills to build rapport with members and coordinate referrals.

Requirements

• Effectively manage and maintain a caseload of assigned ECM members
• Conduct a comprehensive assessment to develop a comprehensive, individualized, person-centered care plan
• Engage with each member and provide culturally appropriate and accessible communication
• Identify necessary clinical and non-clinical resources to assess member health status and gaps in care
• Ensure member's care plan incorporates identified needs and strategies to address needs
