Clinical Team Manager

The Team Manager is responsible for the patient care team, ensuring continuity of care and coordinating with all team members. This role involves direct patient care, team supervision, quality improvement, and regulatory compliance. The position requires a strong understanding of healthcare principles and effective communication skills to enhance team performance.

Requirements

• Reliable transportation with appropriate license and insurance coverage.
• Knowledge of primary medical and nursing care principles and practices.
• Understanding of the interpersonal dynamics within an interdisciplinary health care team.
• Working knowledge of VITAS information system (Vx) and management reports.
• Bachelor's degree in Nursing or related field.
• Current and valid RN License.
