This role is the Lead Patient Care Team Manager for the Patient Care Team, focusing on continuity and quality of care from admission to discharge or transfer. The manager will oversee team schedules, provide direct patient care, and ensure interdisciplinary collaboration and discharge planning. Requirements

• Reliable transportation with proper driver's license and insurance.

• Working knowledge of VITAS information system (Vx).
