As a home care hospice physician, you will use your compassion, communication skills and deep expertise to build meaningful relationships with patients and families, providing the highest-quality care at the most challenging point in life.

Requirements

• Manage approximately 45-60 patients in their home settings via an interdisciplinary team approach.
• Make visits in-person and via telehealth.
• Participate in Weekly team IDG meetings.
• Advocate for patients, discussing goals for their care while addressing their hopes and fears.
• Live the hospice mission, seeking teachable moments everywhere, and educate patients, caregivers, and the communities you serve on the benefits of compassionate, dignified end-of-life care.
