Registered Nurse (RN)

The Registered Nurse, RN, is a member of the interdisciplinary team responsible for providing physical, medical, psychosocial, emotional, and spiritual needs of hospice patients and families. This role involves case management, coordinated care, and providing support through interdisciplinary plan of care. The RN will facilitate communication, documentation, and participation in team meetings.

Requirements

• Current license to practice nursing in the state
• Minimum of two years of nursing experience in hospice, home health, or community health
• Experience in a related field (hospice, home health, or community health) in the last five years
