WestCoast Children’s Clinic is seeking a Care Coordinator and Group Facilitator to provide support and guidance to family and community members of transition-aged youth. The role involves working with a multidisciplinary team to engage and connect clients to appropriate resources and services.
Requirements
- Direct, lived experience with foster care, social services, juvenile justice, mental health, residential treatment or group home placements, wraparound services, or other related community resources.
- Bachelor's degree in Psychology, Criminal Justice, Social Work or related field (preferred)
- At least two (2) years experience working with system-involved youth.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Proven ability to be organized, manage time well, complete high quality and timely documentation.
