Job Description
We are seeking a Case Manager – ACT Team to provide community psychiatric support treatment services for clients with serious and/or persistent mental illness co-occurring with substance abuse disorder. This individual will work with a multidisciplinary team to develop treatment plans and provide wrap-around rehabilitation services.
Requirements
- Collaborate with client and Service Coordinator to develop treatment plans
- Implement programming according to treatment plan
- Assist clients in developing daily living skills
