The Case Manager - Behavioral Health will join a regional treatment team, providing practical help, building community support, teaching recovery-based skills, and monitoring services for clients with severe and persistent mental illnesses.
Requirements
- Serve on a team built up of fellow Case Managers, Recovery Specialists, and medical staff to navigate the needs and care of your clients, attending team meetings on a regular basis.
- Monitor and coordinate the delivery of necessary evaluations and assessments in order to identify each person’s needs.
- Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives.
- Connect clients with community resources to help them develop interpersonal and community coping skills.
- Assist individuals to identify and minimize the effects of their psychiatric symptoms, enabling individuals to self-manage their illness.
- Maintain relevant documentation.
