Job Description
Copa Health is seeking a Case Manager Lead to develop and implement integrated comprehensive assessment and service plans for members, providing ongoing care and coordination with healthcare providers.
Requirements
- Bachelor's degree in behavioral health related field and 2 years' of full-time behavioral health work experience
- Bachelor's degree in non-behavioral health related field and at least 4 years' of full-time behavioral health work experience
- High school diploma or high school equivalency diploma and 6 years' of full-time behavioral health work experience
- Level 1 fingerprint clearance card issued by Arizona Department of Public Safety or eligible to receive within eight (8) weeks of hire
- AZ valid Driver's License, vehicle with state minimum insurance coverage and ability to meet Copa Health company driving requirements and safety sensitive requirements
