Copa Health is seeking a Case Manager Lead to develop and implement integrated comprehensive assessment and Integrated Service Plan (ISP) for members, while providing mentorship and training to new hires and trainees. The role involves coordination with healthcare, behavioral health, and wellness providers, as well as maintaining current documentation on members' progress.
Requirements
- Bachelor's degree in behavioral health related field and 2 years' of full-time behavioral health work experience
- Bachelor's degree in non-behavioral health related field and at least 4 years' of full-time behavioral health work experience
- High school diploma or high school equivalency diploma and 6 years' of full-time behavioral health work experience
- Must have a Level 1 fingerprint clearance card issued by Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire
- AZ valid Driver's License, vehicle with state minimum insurance coverage, and ability to meet Copa Health company driving requirements and safety sensitive requirements
