Lead Case Manager / Metro

Copa Health is seeking a Lead Case Manager to develop and implement integrated comprehensive assessments and service plans, provide ongoing care, and coordinate with healthcare providers. The Lead Case Manager will also provide mentorship and training to new hires and maintain documentation on members' progress.

Requirements

• Bachelor’s degree in behavioral health related field or non-behavioral health related field
• 2 years’ of full-time behavioral health work experience or 4 years’ of full-time behavioral health work experience
• Level 1 fingerprint clearance card or eligible to receive within eight (8) weeks of hire
• AZ valid Driver’s License and vehicle with state minimum insurance coverage
