Case Manager Pathways of Hope

We're seeking a Full-Time Case Manager to join our dynamic healthcare team. The Case Manager participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and provides ongoing care and reviews progress.

Requirements

• High School degree required
• Four (4) years of experience in behavioral health and SMI
• Bachelor's degree in a related field with one (1) year of related experience
• AZ valid Driver's License
• Vehicle with state minimum insurance coverage
• Bilingual (English/Spanish) preferred
