Assessment & Review Practitioner

Assessment & Review Practitioner role involves identifying and completing person-centered care plans, liaising with multi-disciplinary agencies, and ensuring the dignity of service users. The role requires a minimum of SVQ3 in Health and Social Care or equivalent and good organisational and interpersonal skills.
