Job Description
Home Made, Mable, Leap, and Mable Direct, subsidiaries of Attain Healthtech, are seeking passionate Support Partners to empower customers to live independently and safely at home. This role involves building relationships, assessing needs, tailoring support plans, and providing guidance. The position offers a hybrid work environment and requires strong customer service, aged care knowledge, and a consultative approach.
Requirements
- Manage caseload of Home Care Package customers.
- Build confidence and capability for self-management.
- Guide customers through assessments and care plans.
