PRN Physical Therapist, Outpatient Therapy in the Home--Chicago/South Suburbs

The Physical Therapist will provide outpatient quality physical therapy within the patient’s own home and act as a liaison for physician practice to meet the therapy needs of patients. The position offers a high level of flexibility and autonomy to take ownership of a practice and grow a new service line.

Requirements

• One on one in home therapy for post-operative patients
• Devise and execute detailed treatment plans for post-operative patients
• Maintain patient charts, clinical logs and checklists necessary for compliance
• Provide education specific to diagnosis and symptom management needs
• Recognizes and responds to priorities in patient care showing initiative and sound judgment
• Demonstrates willingness to adapt to change in workload and assists with training/orienting others
• Stays abreast of and complies with all state and federal laws including HIPAA, ADA, OSHA, and FLSA
