The Physical Therapist provides outpatient quality physical therapy within the patient’s own home and acts as a liaison for physician practice to meet the therapy needs of patients. The position offers a high level of flexibility as well as autonomy to take ownership of a practice and offers a new standard of post-operative care.
Requirements
- One on one in home therapy for post-operative patients
- Devise and execute detailed treatment plans for post-operative patients
- Maintain friendly, cordial relations with all clients and employees
- Work independently to provide respectful and empathetic patient care
- Cultivate relationships with patients and the community
- Provide continuous communication with referring physicians and patient families
- Maintain patient charts, clinical logs and checklists necessary for compliance
- Set up and monitor home exercise program
- Provide education specific to diagnosis and symptom management needs
- Recognize and respond to priorities in patient care
- Demonstrate willingness to adapt to change in workload and assist with training/orienting others
- Stay abreast of and comply with all state and federal laws
