Job Description
The RN Patient Care Navigator is responsible for case management, care coordination, and utilization management of patients across multiple care levels and settings. The role involves promoting patients' understanding of their diagnosis, treatment options, and available resources, and ensuring they are connected with optimal resources across the continuum of care.
Requirements
- Bachelors Degree Health Administration Required
- Bachelors Degree Nursing Required
- 3 Years Utilization review, discharge planning, case management or disease management preferred
- Nursing experience in home services, ambulatory services working with high-risk patients beneficial
- 2 Years Clinical nursing experience preferred
