Job Description
The RN Patient Care Navigator- Oncology is responsible for guiding high-risk patients and families through the health system, eliminating barriers to access to healthcare services, and facilitating continuity of care. They will establish and document individualized plans of care, partner with the healthcare team, and perform daily coordination between multiple departments and teams.
Requirements
- Bachelors Degree Health Administration Required Or Bachelors Degree Nursing Required
- 3 Years Utilization review, discharge planning, case management or disease management preferred. Nursing experience in home services, ambulatory services working with high-risk patients beneficial.
- 2 Years Clinical nursing experience preferred.
