The RN Patient Care Navigator is responsible for case management, care coordination, and utilization management of patients across multiple care levels and settings. The role promotes patient understanding of their diagnosis, treatment options, and available resources, and ensures smooth and safe care transitions.

Requirements

• Bachelors Degree Health Administration Required Or Bachelors Degree Nursing Required
• 3 Years Utilization review, discharge planning, case management or disease management preferred
• 2 Years Clinical nursing experience preferred
