Job Description
OPO Health is seeking a Medical Social Worker for their home health services in San Francisco County. This role focuses on implementing social work standards of care for patients, providing psychosocial assessments, and collaborating with healthcare systems and community partners. The Social Worker will assist with patient care planning, discharge planning, and support the team to understand psychosocial and emotional factors impacting health issues.
Requirements
- Graduate of a bachelor's program in Social Work accredited by the Council on Social Work Education.
- Licensed as a Clinical Social Worker by the Board of Behavioral Sciences.
- Minimum of one year of experience in health care.
- Experience in Home Health care required.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements.
