Social Services Director

Snowline Health is seeking an experienced, compassionate Social Services Director to lead our social services programs and support our interdisciplinary hospice care team.

Requirements

• Master of Social Work (MSW) required
• Licensed Clinical Social Worker (LCSW) preferred
• Advanced certification (e.g., ACHP-SW) preferred
• Minimum of 5 years of clinical experience in healthcare, hospice, or related field
• Minimum of 3 years of leadership or supervisory experience
• Experience with program development preferred
• Experience supporting patients and families with grief, loss, and end-of-life care
• Strong leadership, communication, and interpersonal skills
• Ability to work effectively with diverse populations
