The Case Manager 1 provides staff support and direct client care, delivering comprehensive case management services to clients including intake assessments, development and implementation of individual care plans, and liaison with treatment providers and community resources.

Requirements

• Some post-secondary education
• 2 years of related experience in the direct service and case management field
• Valid Connecticut driver’s license
