Social Services Case Manager

Westside Family Healthcare is seeking a Social Services Case Manager to join the team. The position involves assessing patient needs and providing access to social services and resources. The case manager will work closely with the primary care, behavioral health, and multidisciplinary teams to support healthcare integration efforts.

Requirements

• Bachelor’s degree in psychology, social work, or another human services area
• At least 3 years of relevant experience
• One year of experience providing services to a culturally diverse population
• Three years of experience using computers and Microsoft Office software
