Social Services Case Manager

Westside Family Healthcare is seeking a Social Services Case Manager to join their team. The Case Manager will assess patients' social, emotional, environmental, financial, and other support needs and connect them to internal services and community resources. The position will work closely with the primary care, behavioral health, and multidisciplinary teams to support healthcare integration efforts.

Requirements

• Bachelor's degree in psychology, social work, or another human services area
• At least 3 years of relevant experience
• One year of experience providing services to a culturally diverse population
• Three years of experience using computers and Microsoft Office software
• Fluency in written and spoken English and spoken Spanish and/or Haitian Creole
